Getting Started is Simple
New Patient Forms
All we’ll need from you to get started is a little new patient paperwork, including some information on your child’s medical and dental history. Simply download your forms ahead of time to complete this before your appointment, or you can fill them out in our office. Aside from your child’s new patient forms, please bring a photo ID and a copy of your insurance card if you plan on using insurance.
